Back when I was young and foolish I was in the habit of taking apart my guns just to see what made them tick.
I took apart a BL-22 and it did the falling out thing. After studying it a while, I put it back together with what I thought was all the parts. Then later I was in a Turkey Shoot with it and the damned thing jammed every shot. I finally figured out that there was supposed to be a little spring under the ejector... So I figured it must have fallen on the floor, which was a shag carpet. I went back to the exact spot where I had taken it apart and after careful combing through the shag carpet, I finally found the damned thing.
